| Summary: | The nursing consultation favors adhesion to the treatment. In case of oral anticoagulation, highlighting the therapy with warfarin, the non-adhesion to treatment is related to the therapeutic failure. Warfarin is commonly recommended when treating cardiovascular pathologies and the frequent monitoring is necessary. PURPOSE: Describe the aspects envisaged in the nursing consultation to ambulatory patients under oral anticoagulation. METHOD: It is an experience report about the actions standardized by the nurses, who perform nursing consultation at an unversity hospital&";s ambulatories. RESULTS: Specific actions to patient were performed: assessment on the patient and companion&";s knowledge, about the disease and treatment proposal; survey on individualized needs and Nursing Diagnostics; pertinent guidance: the warfarin administration time, the drug and food interactions and home care, as well as possible risks on the changed INR (International Normalized Ratio); examination performance to check INR; and control card filling. Although there is no information about the treatment adhesion, it was observed, in the nursing consultations, an opportunity to establish bond with the patients, shown by the trust in the nurse when reporting the real events, which may interfere on the drug dosage to be prescribed by the medical team and on the need of a new specific and individualized approach. DISCUSSION and CONCLUSION: There were many necessary, care measures to the patient under oral anticoagulation, and the individualized approach is pertinent, in order to increase the possibility of therapeutic success. |
