PURPOSE: Describe the aspects envisaged in the nursing consultation to ambulatory patients under oral anticoagulation. METHOD: It is an experience report about the actions standardized by the nurses, who perform nursing consultation at an unversity hospital&";s ambulatories. RESULTS: Specific actions to patient were performed: assessment on the patient and companion&";s knowledge, about the disease and treatment proposal; survey on individualized needs and Nursing Diagnostics; pertinent guidance: the warfarin administration time, the drug and food interactions and home care, as well as possible risks on the changed INR (International Normalized Ratio); examination performance to check INR; and control card filling. Although there is no information about the treatment adhesion, it was observed, in the nursing consultations, an opportunity to establish bond with the patients, shown by the trust in the nurse when reporting the real events, which may interfere on the drug dosage to be prescribed by the medical team and on the need of a new specific and individualized approach. DISCUSSION and CONCLUSION: There were many necessary, care measures to the patient under oral anticoagulation, and the individualized approach is pertinent, in order to increase the possibility of therapeutic success. |

INTRODUÇÃO: A consulta de enfermagem favorece adesão ao tratamento. No caso da anticoagulação oral, em destaque para a terapia com a varfarina, a não adesão ao tratamento está relacionada ao insucesso terapêutico. A varfarina é comumente indicada no tratamento de patologias cardiovasculares e a sua monitorização frequente é necessária. OBJETIVO: Descrever os aspectos contemplados na consulta de enfermagem a pacientes ambulatoriais sob anticoagulação oral. MÉTODO: Trata-se de um relato de experiência sobre as ações padronizadas pelos enfermeiros que realizam consulta de enfermagem nos ambulatórios de um hospital universitário. RESULTADOS: Ações específicas ao paciente foram realizadas: avaliação do seu conhecimento e do acompanhante, sobre a doença e proposta de tratamento; levantamento das necessidades individualizadas e dos Diagnósticos de Enfermagem; orientações pertinentes: ao horário da administração da varfarina, as interações medicamentosas e alimentares e os cuidados domiciliares, bem como possíveis riscos na vigência de INR (Razão Normalizada Internacional) alterado; realização do exame para checagem do INR; e preenchimento do cartão de controle. Embora não se tenham dados quanto a adesão ao tratamento, observou-se nas consultas de enfermagem uma oportunidade para se estabelecer vínculo com os pacientes, demonstrada pela confiança no enfermeiro em relatar os acontecimentos reais, que podem interferir na dosagem da medicação a ser prescrita pela equipe médica e na necessidade de nova abordagem específica e individualizada. DISCUSSÃO e CONCLUSÃO: Muitos foram os cuidados necessários ao paciente sob anticoagulação oral, e a sua abordagem individualizada é pertinente a fim de aumentar a possibilidade de sucesso terapêutico.
